What causes scale build-up in pool water?

Scale occurs when minerals in the water (especially calcium) crystallise and stick to surfaces. This is commonly caused by high pH, high calcium hardness, or warm water. Scale can build up on pool tiles, walls, heaters, and chlorinator cells.
